The Care Management Support Assistant 2 contributes to the administration of care management by providing non-clinical support to the assessment and evaluation of members' needs. This support helps achieve and maintain an optimal wellness state by guiding members/families toward resources appropriate for their care and wellbeing. Additionally, you will triage different risk assessments and assign them to the care management team. You will also follow up with the team inbox and direct referrals to the appropriate queue or tasks those to a care manager. Furthermore, you will complete health risk assessments to identify needs, which will be assigned to the appropriate care management. Be comfortable with a high volume of calls and perform outreach to members on a regular basis. You will also assist members by helping them understand their benefits and connect them to resources available. You will report to a Manager, Care Management.
