The Care Management Support Coordinator position is a non-clinician role that works in collaboration with the Care Management Team to support the multi-disciplinary team approach of patient care. Care Management Coordination supports both members and the CM (Care Management) Team by providing resources, requesting, and gathering appropriate documents, scheduling members for Care Manager follow up, and completing other delegated tasks to support the CM Team. The Care Management Support Coordinator will work remotely within regions to cover needs across the state.
