The Behavioral Health Care Management Specialist - ECM will support enhanced care management (ECM) patients with behavioral health conditions, severe mental illnesses (SMI), and substance use disorders (SUD) needs using brief interventions and behavioral activation strategies. This role will provide care coordination and connection to services and social supports for ECM patients, including appointment scheduling and referral management. This is a hybrid position; on site expectations are dependent on performance. Proposed schedule shifts: Mondays through Fridays 8:30 AM - 5 PM (30-min lunch), or 9 AM - 6 PM (1-hour lunch).
