The Enhanced Care Management (ECM) Program Manager provides leadership and direction for the organization’s care management operations and services, aligning with the organization's mission, strategies, and objectives. This role supervises the care management team and leads the implementation of care management programs designed to address patient needs, improve care quality and services, and ensure appropriate utilization of available services. The Program Manager implements plans and tools to meet organizational goals, collaborates with other departments to design, implement, and evaluate care management programs, and leads complex projects focused on performance improvement and transformation. They support the development and evaluation of effective pilots and programs based on research and performance outcomes, ensuring standardization and optimization of workflows. Additionally, the role involves developing playbooks for care management models, performing patient risk stratification, organizing stakeholder meetings, evaluating research, and working on the design and implementation of IT solutions to support care management documentation and monitoring. The Program Manager also facilitates the deployment of an administrative case management tracking system and performs miscellaneous job-related duties as assigned.
