The Enhanced Care Management (ECM) Program Manager provides leadership and direction for the organization’s care management operations and services, aligning with the organization's mission, strategies, and objectives. This role supervises the care management team and leads the implementation of care management programs designed to address patient needs, improve care quality, and ensure appropriate utilization of available services. The Program Manager implements plans and tools to meet organizational goals, works closely with other departments to design, implement, and evaluate care management programs, and leads complex projects focused on performance improvement and transformation. They support the development, implementation, and evaluation of effective pilots and programs derived from market-leading research and performance outcomes, ensuring standardization and optimization of workflows for scaling models of care. The role involves developing playbooks for scalable care management models, working with clinical and operational leaders for patient risk stratification, and organizing meetings with key stakeholders. Additionally, the Program Manager continuously evaluates research focused on care management programs and works on the design, implementation, and evaluation of IT solutions to support care management documentation and monitoring. They facilitate and lead the deployment of an administrative case management tracking system/care management documentation. This position reports to the Director of Operations and performs miscellaneous job-related duties as assigned.
