About The Position

St. Mary's Medical Center is seeking a Casual Care Facilitator for its Care Management Department. The Care Facilitator exercises independent judgment and professional discretion in case management for patients. The role collaborates with attending physicians and residents and advocates for patients and families as they navigate an acute episode of care. The Care Facilitator monitors patients’ continuing stays for resource utilization and helps prepare them for safe and timely discharge. The role uses proactive, collaborative problem-solving to meet patients’ and physicians’ needs.

Requirements

• Graduate of an approved school of nursing.
• Hold and maintain a current Registered Nursing License for the state of West Virginia.
• Five years of nursing experience, including hospital patient care, direct knowledge of care management, discharge planning, and resource management.
• Current BLS certification.

Nice To Haves

• BSN or master’s degree.
• Certification in Care Management.
