Care Coordinator
About The Position
Grandvue Medical Care Facility is seeking an RN to join its state-of-the-art facility as a Care Coordinator. The successful candidate will bring strong clinical and assessment skills, experience coordinating resident care, leadership, communication, and team-building abilities, and a positive, dynamic, evidence-based approach to nursing with a passion for improving residents’ lives. Long-term care, dementia, and MDS experience and certification are preferred but not required; the facility will train the right candidate. The role offers opportunities to build rewarding relationships with care partners.
Requirements
- Must be an RN.
- Strong clinical and assessment skills.
- Experience coordinating resident care.
- Strong leadership, communication, and team-building skills.
- Positive, dynamic, evidence-based nursing approach and a passion for improving residents’ lives.
- Annual influenza vaccination or a GMCF-approved exemption is required for employment.
