About The Position

Sentara Health is hiring a Full Time Care Coordinator for the South Central Richmond, VA area! This position requires travel for in person face-to-face assessments in around the south Central Richmond- Dinwiddie, Petersburg, Prince George, SouthHill. Days without assessments scheduled, the employee can work remotely. Applicants must be located in the south Central Richmond Area of VA.

Requirements

  • Bachelor's degree in Health & Human Services or one of the following related Fields: Art Therapy, Behavioral Sciences, Child Development, Cognitive Sciences, Community Mental Health Counseling (MH, Vocational, Pastoral, etc.), Counselor Education, Early Childhood Development, Educational Psychology, Gerontology, Healthcare Administration, Human Development, Human Services, Marriage and Family Therapy, Music Therapy, Nursing, Pharmacy, Psychiatric Rehabilitation, Psychology, Rehabilitation Counseling, Social Work, Sociology, Special Education, Speech Therapy, Therapeutic Recreation, Vocational Rehabilitation.
  • 1 year of social work, care coordination, case management, behavioral health, healthcare, or community-based services experience.
  • Must be located in the south Central Richmond Area of VA.

Nice To Haves

  • Experience supporting individuals with Intellectual and Developmental Disabilities (ID/DD) and waiver services.
  • Experience working with individuals affected by substance use disorders and coordinating treatment or recovery support services.
  • Experience advocating for patients, clients, or vulnerable populations and assisting with care transitions and service navigation.

Responsibilities

  • Conduct in-person face-to-face assessments in the south Central Richmond area (Dinwiddie, Petersburg, Prince George, South Hill).
  • Work remotely on days without scheduled assessments.
  • Provide care coordination, case management, and behavioral health services.
