About The Position
Sentara Health is hiring a Full Time Care Coordinator for the Hampton Road area! This position requires travel for in person face-to-face assessments in the Hampton Roads- Newport News area. Days without assessments scheduled, the employee can work remotely. Applicants must be located in the Newport News area.
Requirements
- Bachelor's degree in Health & Human Services or one of the following related Fields: Art Therapy, Behavioral Sciences, Child Development, Cognitive Sciences, Community Mental Health Counseling (MH, Vocational, Pastoral, etc.), Counselor Education, Early Childhood Development, Educational Psychology, Gerontology, Healthcare Administration, Human Development, Human Services, Marriage and Family Therapy, Music Therapy, Nursing, Pharmacy, Psychiatric Rehabilitation, Psychology, Rehabilitation Counseling, Social Work, Sociology, Special Education, Speech Therapy, Therapeutic Recreation, Vocational Rehabilitation
- 1 year of social work, care coordination, case management, behavioral health, healthcare, or community-based services experience

- Experience conducting psychosocial, or needs-based assessments and developing individualized care plans
- Experience advocating for patients, clients, or vulnerable populations and assisting with care transitions and service navigation
