About The Position

The Care Coordinator educates patients about their health conditions and develops plans to address their personal healthcare needs, with a focus on coordinating care for patients with substance abuse health needs. The role works closely with providers, case managers, social workers, and other departments to help patients attend scheduled appointments, obtain needed care, and overcome barriers as they transition from the hospital to their next level of care. The coordinator provides service coordination and support based on assessed needs and physician recommendations.

Requirements

At least 1 year of previous experience.
BLS certification required.
CPI certification required within 30 days of hire.

Nice To Haves

Bilingual in Spanish preferred.

Responsibilities

Provide face-to-face psychosocial assessments for patients with substance abuse needs.
Review and update completed assessments daily and perform other duties assigned by the program director.
Facilitate referrals for additional services consistent with assessed needs and physician recommendations.
Meet with patients for follow-up reviews or updates as needed or required.
Provide brief interventions, stabilization services, and supportive counseling to patients.
Under the program director's direction, provide medication education printouts focused on understanding illness, symptom management, and medication compliance.
Complete and file required documentation in a timely manner.
