The Care Coordinator generally meets with the client(s) and the client’s family (foster, adoptive or biological) in the client’s home or residence, on a regularly scheduled basis with Phone call triage assessments during the off months and weeks of face-to-face care. In addition, the Care Coordinator may meet with the members of the client’s extended family and community supports at locations such as school, court, job site, etc., as specified to meet the client’s service and advocacy needs. The Care Coordinator’s hours of work will vary depending on the individual needs of his/her caseload but are generally able to be scheduled in a manner that does not exceed 40 hours in a given week.
