About The Position

Our mission at Apicha Community Health Center is to provide high quality, equitable, whole person and culturally responsive care delivered in an inclusive and welcoming manner. We are currently seeking a Care Coordinator in our 400 Broadway location. The Care Coordinator will lead a team of Patient Navigators and Patient Assistants to ensure care coordination services are provided to all clients assigned to meet their needs for better health outcomes. This role involves day-to-day supervision of the Patient Navigators and Patient Assistant through weekly schedule monitoring, chart review, face-to-face case discussions, and performance reviews. The Care Coordinator is also responsible for the implementation of service/care plans, supported by Patient Navigators, and will conduct outreach and recruitment for Apicha CHC to promote the agency services.

Requirements

  • BA/BS in related field
  • Two (2) years of qualifying experiences
  • Ability to adapt changing environment
  • Appreciation of and commitment to Apicha’s mission and values
  • Excellent MS Office skills (Outlook, Word, Excel, PowerPoint)
  • Ability to work effectively in both a collaborative as well as individual setting
  • Must possess ability to interact with all levels of external and internal customers with a high level of professionalism and tact in representing Apicha CHC
  • Excellent customer service skills to assist in resolving member complaints
  • Strong verbal and written communication skills – for both internal and external audience

Nice To Haves

  • An advanced degree in one of the qualifying fields may be substituted for one (1) year of qualifying experience
  • Experience working with target populations strongly preferred (HIV/AIDS who are virally unsuppressed, individuals with serious mental illness, serious substance use disorders (SUDS), and/or chronic hepatitis)
