About The Position

The Care Coordinator is a key member of the Behavioral Health Team and serves as an advocate for patients by coordinating care to minimize the wait for services, effectively assess needs of patients, and increase engagement in therapy and integrated services. They directly support evolvedMD’s Behavioral Health Managers (BHMs) by promoting effective engagement of patients referred for external support services.

Requirements

  • Bachelor’s degree in a clinical behavioral health field (i.e., counseling, social work), preferred.
  • Two+ years of experience in case management, care coordination and/or behavioral health.
  • Potential for competency in multiple Electronic Heath Record Systems
  • Proven + Passionate Drive – A love for and commitment to patient impact while also driving clinical outcomes
  • Collaboration Dedication – The desire to work as a team with a diverse set of behavioral and physical health providers to ensure “Whole Patient Care, Upfront and Ongoing.
  • Ability to manage one's own time efficiently.

Responsibilities

  • Collaborate with BHMs both in-person and virtually to assess patient needs, support the implementation and follow-up of care plans, and contribute to team-based treatment planning within the Collaborative Care Model.
  • Engage directly with patients, both face-to-face and telephonically, to provide education, emotional support, and assistance in accessing external community and behavioral health resources; serve as a patient advocate and care navigator.
  • Utilize knowledge of common mental health disorders to deliver informed care, including timely follow-ups, crisis intervention, and coordination during discharges from behavioral health hospitalizations.
  • Maintain accurate and timely documentation of all patient interactions in electronic health systems, ensuring compliance with HIPAA and organizational standards for reporting and confidentiality.
  • Work independently in a fast-paced clinic setting and remote environment while continuously building expertise in the CoCM and promoting continuity and quality of care across services.
