About The Position

The Care Coordinator is key personnel responsible for managing admissions for the facility. The role assists the clinical management team by overseeing treatment plans and collaborating with other team members to ensure residents receive care through assessments, treatments, monitoring, resident-goal development, and communication about progress. The Care Coordinator also supports staff in providing quality care to residents.

Requirements

• Must be a licensed LPN.
• Must have any necessary required certifications where needed.
• Must have a high level of computer skills and PCC knowledge.
• Demonstrate the ability and willingness to work harmoniously with Department Heads, personnel, ODH staff, and residents.
• Must deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
• Must have patience, a cheerful disposition, and enthusiasm when handling difficult situations.
• Must have leadership and excellent communication skills.
• Must be able to make independent decisions when circumstances warrant such action.
• Must be willing to seek out new methods and principles and incorporate them into existing policies and procedures.
• Must be open to change and meet the requests of Administration and Nursing.
• Must communicate effectively with Administration, peers, and staff.
