Person Centered Services Care Coordination Organization is seeking Care Coordinators to serve people with intellectual and developmental disabilities within a health home model. The role coordinates and advocates for all aspects of each individual’s care, including health and behavioral healthcare, community supports, and other services needed to meet their needs. Care Coordinators use a person-centered, holistic, strengths-based approach and collaborate with the individual’s healthcare and service providers to help ensure the person’s wants and needs are met. The role is full-time, Monday through Friday, with an eight-hour shift and no nights. Care Coordinators meet individuals in their homes, provider offices, and other public places to conduct assessments and provide services. Work is independent from home, with office attendance at least once per week and potentially more often based on business needs. During the first 90 days, three onsite days per week are required for onboarding and integration; after successfully completing that period, team members may transition to a hybrid schedule. Supervisors may extend the onsite requirement as needed. New team members participate in New Employee Orientation, offered Monday through Friday onsite and online. Day one includes in-person training at the West Seneca, New York office. Social workers, care managers, and case managers are encouraged to apply.
