About The Position

Endeavor DDD Services is hiring a Care Coordinator in Mesa to support individuals with intellectual developmental disabilities across the East Valley. This role involves being the connection between families, providers, and services to ensure members receive the highest quality care. Requirements

• Experience in HCBS / DDD / Case Management / DSP roles
• Knowledge of Arizona DDD programs and services
• HIPAA compliance and confidentiality awareness
• Strong computer skills (Google Workspace, Apple preferred)
• Valid driver's license & reliable transportation
• Ability to pass APS & CPS registry checks

Nice To Haves

• Experience in HCBS, DDD services, case management, or direct care
• Group Home Management or Direct Support Professional (DSP) roles
• Strong communication & relationship-building skills
• Organized, detail-oriented, and solutions-driven
• Understand Arizona DDD systems and service delivery

Responsibilities

• Manage and support a caseload of DDD members
• Conduct home visits and build strong relationships with families
• Coordinate Attendant Care, Habilitation, and Respite services
• Advocate for member needs and ensure services align with PCSPs
• Collaborate with DSPs, families, and support teams
• Monitor service utilization, compliance, and outcomes
