About The Position

As a Care Coordinator at Endeavor DDD Services, you will be the vital link between families, providers, and essential services, ensuring that members with intellectual and developmental disabilities receive the highest quality of care. This role involves managing a caseload, conducting home visits, coordinating various support services, advocating for member needs, and collaborating with a multidisciplinary team to ensure services align with individual plans and achieve positive outcomes. Requirements

• Experience in HCBS / DDD / Case Management / DSP roles
• Knowledge of Arizona DDD programs and services
• HIPAA compliance and confidentiality awareness
• Strong computer skills (Google Workspace, Apple preferred)
• Valid driver's license & reliable transportation
• Ability to pass APS & CPS registry checks

Nice To Haves

• Experience in Group Home Management or Direct Support Professional (DSP) roles
• Strong communication & relationship-building skills
• Organized, detail-oriented, and solutions-driven
• Understanding of Arizona DDD systems and service delivery
• Passionate about helping individuals with developmental disabilities live meaningful lives

Responsibilities

• Manage and support a caseload of DDD members
• Conduct home visits and build strong relationships with families
• Coordinate Attendant Care, Habilitation, and Respite services
• Advocate for member needs and ensure services align with PCSPs
• Collaborate with DSPs, families, and support teams
• Monitor service utilization, compliance, and outcomes
