About The Position

Grandvue Medical Care Facility is seeking an RN for a leadership opportunity as a Care Coordinator in its state-of-the-art facility. The successful candidate will bring clinical and assessment skills, experience coordinating resident care, leadership and communication abilities, team-building skills, and a positive, dynamic, evidence-based approach to improving residents’ lives. The facility will train the right candidate.

Long-term care, dementia, and MDS experience and certification are preferred but not required. Annual influenza vaccination or a facility-approved exemption is required for employment; COVID-19 vaccinations are highly encouraged.

Requirements

• RN.
• Strong clinical and assessment skills.
• Experience coordinating resident care.
• Strong leadership, communication, and team-building skills.
• Positive, dynamic, evidence-based approach and passion for improving residents’ lives.
• Annual influenza vaccination or a Grandvue Medical Care Facility-approved exemption required for employment.
