About The Position

HomeWell Care Services is seeking a compassionate, highly organized, and dedicated Care Coordinator to join their team. This role serves as the primary point of contact for clients and caregivers, helping coordinate services, develop individualized care plans, and ensuring clients receive the support they need. Exceptional written and verbal communications skills are a must as well as having the ability to interact effectively with professionals in a variety of settings including medical settings and the community. Must be passionate about helping seniors live with dignity.

Requirements

  • Minimum of one year of experience in healthcare, social work, or a related field.
  • Thrive in a fast-paced environment and demonstrate exceptional multitasking abilities while maintaining a high level of accuracy and attention to detail.
  • Outstanding planning and organizational skills.
  • Strong critical thinking and problem-solving abilities.
  • Ability to effectively prioritize competing responsibilities.
  • Proficient in the Microsoft Office Suite and comfortable using computers and electronic systems in a professional healthcare setting.
  • Experience working in a healthcare setting is required.
  • Exceptional multitasking, organizational, and time-management skills with the ability to manage competing priorities in a fast-paced environment.
  • Strong verbal and written communication skills.
  • Proficiency with electronic health records (EHR) and Microsoft Office applications.
  • Valid driver's license and reliable transportation.
  • Must be able to pass a national/expanded background check as required by the State of Indiana and drug screen.
  • Must live in and be authorized to work in the United States of America.

Nice To Haves

  • Previous scheduling experience is preferred but not required.
  • Experience in care coordination, case management, nursing, medical offices, behavioral health, or social services is strongly preferred.
  • Degree in healthcare or Social Work or a related field is preferred but not required with relevant experience.

Responsibilities

  • Coordinate and monitor individualized care plans to ensure clients receive appropriate, timely services.
  • Conduct client assessments and follow up.
  • Communicate effectively with caregivers, clients, families, healthcare providers, and community partners.
  • Maintain after hours and weekend mobile phone coverage.
  • Schedule appointments, coordinate referrals, and facilitate communication between providers.
  • Maintain accurate, timely, and confidential documentation in compliance with organizational and regulatory standards.
  • Monitor client progress and update care plans as needed.
  • Conduct supervisory visits as needed.
  • Manage multiple client cases simultaneously while prioritizing tasks and meeting deadlines.
  • Advocate for clients to ensure they receive comprehensive, person-centered care.
  • Complete scheduling as directed by management.
  • Participate in interdisciplinary team meetings and collaborate with healthcare professionals to support positive client outcomes.
  • Be a team-player and assist in other tasks as required by management.
