This position is responsible for integrated patient care coordination under the direction of the Integrate Care Team model. The role involves optimizing and pulling data to identify high-risk patients, administering health screens, educating patients and families, and coordinating care with internal and external professionals. The Care Coordinator will also manage appointment scheduling, maintain patient records, and represent the organization in the community. A key aspect of the role is documenting and coordinating data across multiple EMRs and databases, as well as coordinating discharge and transition planning. The position requires performing assessments of medical and behavioral healthcare needs, family support structures, and patient eligibility for resources, while advocating for patients' healthcare choices and communicating barriers to healthcare teams. Knowledge of Medicaid/Medicare guidelines, Marketplace plans, and commercial insurance benefits is essential. Experience with MCE plan benefits is highly preferred. The role also includes performing other related duties as directed or required.
