Cayuga Health System is seeking an energetic and motivated individual in search of part-time, benefited, employment in this newly created position. The Care Coordinator will play an integral role in a number of activities in the promotion of safe patient care transition. The care coordinator collaborates with providers & practices (inpatient and outpatient), hospitals, home care agencies and additional community-based organizations to assure collaborative and consistent patient care delivery. Applicant should have strong analytical, problem solving, critical thinking, and organizational skills. The Care Coordinator must possess the ability to function independently and have the flexibility and personal integrity to work in partnership with system stakeholders and partners as assigned. The Care Coordinator also serves the needs of the department to optimize efficiencies for team members and supervisors. Must be able to prioritize, coordinate, and simultaneously maintain multiple projects with high level of quality and productivity.
