About The Position

Fairview is seeking a Care Coordinator Registered Nurse to join its Clinics and Surgery Center in Minneapolis, Minnesota, primarily working at the Maple Grove Specialty Clinic in Maple Grove, Minnesota. This is a full-time, salaried position at 1.0 FTE, with Monday-through-Friday, eight-hour shifts scheduled between 8:00 a.m. and 5:00 p.m. The role specializes in wound care in an ambulatory clinical setting. The Care Coordinator provides clinical guidance, education, and support to patients, families, medical providers, and healthcare staff. The position promotes optimal wound healing and prevention of complications through evidence-based assessment, treatment, and management. It coordinates patient-centered care across the continuum, serves as a clinical resource and liaison, and helps establish individualized treatment plans that promote continuity of care and positive wound-healing outcomes.

Requirements

Licensed Registered Nurse with a four-year bachelor's degree.
Minnesota Registered Nurse (RN) license.
Basic Life Support certification from the American Heart Association or Red Cross; this requirement does not apply to department 8901 Ambulatory Care Coordination within 30 days.

Nice To Haves

Two years of specialty experience in care coordination; or five years as a licensed RN with five years of clinical experience and the ability to learn a new specialty area.
Specialized knowledge of the specialty and conditions of the population served.

Responsibilities

Access, analyze, plan, and provide follow-up care.
Facilitate timely referrals and foster positive relationships with community providers.
Evaluate and document patient status, follow progress through phone calls and test results, prepare reports, and evaluate nursing-care quality through patient rounds.
Identify problems specific to the clinical specialty and provide proactive disease management using evidence-based practices.
Liaise among patients, families, medical staff, and others involved in treatment to promote patient-centered care based on jointly determined goals and priorities.
Collaborate with staff to coordinate transitions of care across the healthcare continuum.
Provide direct patient care in the specialty area and establish individual nursing care plans.
Provide education, counseling, medication management, and general health information to patients and their representatives.
Triage patient concerns and respond between visits under provider direction to reduce relapse or inpatient care.
Serve as a clinical-specialty resource to patient-care units, outpatient clinics, and medical staff.
Identify needs and assist with writing and implementing policies and procedures.
Educate patients, families, and hospital staff about the clinical specialty; develop content and resource materials and coordinate educational plans.
Serve on task forces and committees related to the clinical specialty.
Keep current on technological advances and new pharmaceutical products; consult with other areas regarding the specialty.
Assist with research projects, including data collection, analysis, and evaluation, and potentially identify research projects.
Manage databases tracking patient outcomes and other categories as directed by a physician.
Promote the program and assist with marketing; oversee or manage prior authorizations and results verification to support a positive image.
Provide effective educational materials and serve as a community liaison.
Perform other duties, including performance goals established by the manager and employee and documented in Fairview's performance-management tools.
Develop and maintain competencies listed in the job description and the employee's online performance-management tool.
