Provide individualized, therapeutic-based support to youth and young adults (YYAs) experiencing homelessness. Support YYAs throughout the full continuum of care while engaged in Daybreak’s programs. Help YYA’s transition from shelter to securing safe and stable housing while connecting them to programming and resources that promote long-term stability and overall well-being. Work closely with clients to create individualized service plans that emphasize self-sufficiency, community integration, and holistic care, tailored to each client’s unique needs and strengths. The Care Coordinator builds trusting relationships with YYAs and collaborates with service providers to connect clients to housing, employment, healthcare, and personal development opportunities. The Care Coordinator is a strong communicator with heightened cultural competence, persistence, and a passion for helping young people move from homelessness to independence and stability.
