Ennoble Care is a mobile primary care, palliative care, and hospice service provider operating in multiple states, including Georgia. Their clinicians provide continuum of care in patients' homes for those with chronic conditions and limited mobility, offering programs like remote patient monitoring, behavioral health management, and chronic care management. Ennoble Care is seeking a full-time Embedded Care Coordinator to provide on-site support within partnered long-term care facilities in the Atlanta, Georgia area, specifically focusing on Fayetteville. This role is an integral, in-person member of the clinical care team, working alongside Ennoble Care providers in a family-practice-style model. The "Embedded" aspect means the coordinator is physically on-site, building consistent relationships with residents and families, and becoming a trusted presence within the facility to support care, similar to a traditional family practice in a long-term care setting. The work focuses on a dedicated group of patients, with a strong emphasis on remote patient monitoring (RPM) and proactive care coordination.

Requirements

  • Legally authorized to work in the United States of America
  • Active, unrestricted clinical certification or licensure, such as: Patient Care Technician (PCT), Certified Nursing Assistant (CNA), Medical Assistant (MA) / Certified Medical Assistant (CMA), Licensed Practical Nurse (LPN), Registered Nurse (RN), Or comparable credential
  • Minimum 1 year of patient-facing experience in a long-term care, skilled nursing, or similar clinical setting
  • Comfortable communicating proactively and professionally throughout the day via Microsoft Teams, email, phone, and in person
  • Cultivate and maintain positive, approachable relationships with residents, families, providers, and facility staff, while demonstrating a consistent, dependable presence within the long-term care setting
  • Well-developed organizational and time-management skills, paired with an optimistic approach and initiative-driven mindset
  • Strong working knowledge of healthcare terminology and care coordination principles
  • Proficiency with Microsoft 365, including Outlook, Word, and Excel
  • Ability to work full-time, Monday through Friday, 8:30 a.m. to 5:00 p.m. EST, (or thereabouts, contingent on facility hours) in an on-site role, based in the Atlanta, Georgia area, with a primary focus on the Fayetteville vicinity
  • No weekends
  • No on-call
  • Daytime hours
  • Seven (7) company-paid holidays annually
  • Valid, unencumbered driver’s license
  • Consistent, reliable transportation to fulfill the duties of this position
  • Current auto insurance, with the ability to provide proof
  • Ability to pass a background check and drug screen

Responsibilities

  • Coordinate care for long-term care residents using Ennoble Care’s EMR
  • Maintain regular, proactive, on-site engagement with residents to support care plans, follow-ups, and care transitions
  • Collaborate closely with Ennoble Care providers, within the long-term care setting
  • Serve as a key liaison between providers, facility staff, residents, and families
  • Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely manner
  • Accurately document patient interactions and care coordination activities
  • Follow established workflows, documentation standards, and escalation protocols
  • Contribute positively to team collaboration and strong facility partnerships
