Their clinicians provide a continuum of care in patients' homes, focusing on those with chronic conditions and limited mobility. Ennoble Care offers various programs such as remote patient monitoring, behavioral health management, and chronic care management, aiming to deliver high-quality care through a trusted team. The company seeks individuals driven to make a difference, embodying their motto, “To Care is an Honor.” Ennoble Care is hiring a full-time Care Coordinator, Embedded, to provide on-site support within one or more partnered long-term care facilities in the Atlanta, Georgia area, specifically in the Roswell vicinity. This in-person role is a vital part of the clinical care team, working alongside Ennoble Care providers in a family-practice-style model that prioritizes continuity, relationship-building, and patient-centered support. As an Embedded Care Coordinator, the individual will be physically present in long-term care facilities, collaborating with providers and facility staff. This role involves building consistent relationships with residents and families, becoming a familiar and trusted presence, and supporting care in a manner similar to a traditional family practice within the long-term care setting. The work is heavily focused on family-practice principles, including a strong emphasis on remote patient monitoring (RPM) and proactive care coordination. Requirements

  • Legally authorized to work in the United States of America
  • Active, unrestricted clinical certification or licensure, such as: Patient Care Technician (PCT), Certified Nursing Assistant (CNA), Medical Assistant (MA) / Certified Medical Assistant (CMA), Licensed Practical Nurse (LPN), Registered Nurse (RN), Or comparable credential
  • Minimum 1 year of patient-facing experience in a long-term care, skilled nursing, or similar clinical setting
  • Comfortable communicating proactively and professionally throughout the day via Microsoft Teams, email, phone, and in person
  • Cultivate and maintain positive, approachable relationships with residents, families, providers, and facility staff, while demonstrating a consistent, dependable presence within the long-term care setting
  • Well-developed organizational and time-management skills, paired with an optimistic approach and initiative-driven mindset
  • Strong working knowledge of healthcare terminology and care coordination principles
  • Proficiency with Microsoft 365, including Outlook, Word, and Excel
  • Ability to work full-time, Monday through Friday, 8:30 a.m. to 5:00 p.m. EST, (or thereabouts, contingent on facility hours) in an on-site role, based in the Atlanta, Georgia area, with a primary focus on the Roswell vicinity.
  • Valid, unencumbered driver’s license
  • Consistent, reliable transportation to fulfill the duties of this position
  • Current auto insurance, with the ability to provide proof
  • Ability to pass a background check and drug screen

Responsibilities

  • Coordinate care for long-term care residents using Ennoble Care’s EMR
  • Maintain regular, proactive, on-site engagement with residents to support care plans, follow-ups, and care transitions
  • Collaborate closely with Ennoble Care providers, within the long-term care setting
  • Serve as a key liaison between providers, facility staff, residents, and families
  • Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely manner
  • Accurately document patient interactions and care coordination activities
  • Follow established workflows, documentation standards, and escalation protocols
  • Contribute positively to team collaboration and strong facility partnerships
