Ennoble Care is seeking a full-time, embedded Care Coordinator to provide on-site support within one or more partnered long-term care facilities in the Atlanta, Georgia area, specifically focusing on Dunwoody. This role is an integral, in-person member of the clinical care team, working alongside Ennoble Care providers in a family-practice-style model. The position emphasizes continuity, relationship-building, and patient-centered support. As an Embedded Care Coordinator, the individual will be physically present in long-term care facilities, collaborating with providers and facility staff. This role involves building consistent relationships with residents and families, becoming a familiar presence, and supporting care in a manner similar to a traditional family practice within the long-term care setting. The work is heavily focused on family practice, including remote patient monitoring (RPM) and proactive care coordination for a dedicated group of patients.
