The Care Coordination Program Manager is based out of the Steamboat or Craig office. The Health Partnership is seeking a dynamic and collaborative leader to oversee and strengthen its Care Coordination Program. The Care Coordination team plays a vital role in helping community members across the Yampa Valley access the healthcare, support services, and resources they need to thrive. This position provides leadership and support to the Care Coordination team while building strong partnerships with healthcare providers, community organizations, and other stakeholders to improve care access and advance health equity in rural communities. Supervision Exercised: The Care Coordination Program Manager supervises the Care Coordination and Behavioral Health Care Coordination teams. Supervision Received: The Care Coordination Program Manager is supervised by the Direct Services Director. The Role: The Care Coordination Program Manager is based out of the Steamboat or Craig office and is responsible for leading the Care Coordination team and providing programmatic oversight in collaboration with regional partners across multiple counties, as well as with our state partner, Rocky Mountain Health Plans. This position requires strong leadership, project management, collaboration, and interpersonal skills, along with a deep commitment to supporting individuals and communities.
