About The Position

The Care Coordination Program Manager is based out of the Steamboat or Craig office. The Health Partnership is seeking a dynamic and collaborative leader to oversee and strengthen its Care Coordination Program. The Care Coordination team plays a vital role in helping community members across the Yampa Valley access the healthcare, support services, and resources they need to thrive. This position provides leadership and support to the Care Coordination team while building strong partnerships with healthcare providers, community organizations, and other stakeholders to improve care access and advance health equity in rural communities. Supervision Exercised: The Care Coordination Program Manager supervises the Care Coordination and Behavioral Health Care Coordination teams. Supervision Received: The Care Coordination Program Manager is supervised by the Direct Services Director. The Role: The Care Coordination Program Manager is based out of the Steamboat or Craig office and is responsible for leading the Care Coordination team and providing programmatic oversight in collaboration with regional partners across multiple counties, as well as with our state partner, Rocky Mountain Health Plans. This position requires strong leadership, project management, collaboration, and interpersonal skills, along with a deep commitment to supporting individuals and communities. The ideal candidate will bring exceptional organizational and management abilities, strategic thinking, and an innovative mindset to ensure programs are effectively supported and consistently deliver high-quality services with measurable impact and excellence.

Requirements

  • Bachelor’s degree or relevant experience required.
  • Minimum of one or two years of clinical and/or care coordination experience, including experience supervising or managing employees.
  • Strong background in customer service, case management, care coordination, or related human services work preferred.
  • Education or relevant experience demonstrating strong analytical skills, including data tracking, reporting, documentation accuracy, program metric review, and process improvement.
  • Experience maintaining accurate, confidential documentation and reporting, and supporting HIPAA-compliant practices preferred.
  • Strong problem-solving and analytical skills, with the ability to improve processes, streamline systems, and support operational efficiency.
  • Ability to engage and collaborate with diverse stakeholders to advance shared community goals and initiatives.
  • Skilled in reviewing, interpreting, and utilizing data to inform decision-making and program management.
  • Demonstrated ability to build and maintain positive, professional relationships with colleagues, partners, and community members.
  • Experience with conflict resolution, employee coaching, performance management, and disciplinary processes.
  • Knowledge of healthcare delivery systems and care coordination models preferred.
  • Ability to understand and manage budgets, financial reports, and program-related expenditures.
  • Highly organized with strong attention to detail, follow-through, and the ability to manage multiple priorities effectively.
  • Experience identifying, tracking, and leveraging data to strengthen collaboration, outcomes, and program impact.
  • Proficient in Microsoft Office Suite and database management systems.
  • Experience working on cross-functional or collaborative projects preferred.

  • Ability to travel independently throughout the region, including during inclement weather conditions.
  • Must possess a valid driver’s license and maintain automobile insurance coverage of at least $300,000 per occurrence.

Responsibilities

  • Ensure high quality, effective, and accessible delivery of care coordination services across the counties we serve.
  • Provide oversight of care coordination services, ensuring alignment with Rocky Mountain Health Plans (RMHP) including: Track and monitor care coordination client referrals in alignment with RMHP requirements. Ensure accuracy in care coordination documentation. Develop a network of subspecialty care providers and community resources. Create protocols and procedures to evaluate the effectiveness of referrals.
  • Performs all other special duties as assigned and requested.
  • Ensure long-term financial sustainability of the care coordination program by meeting all program deliverables.
  • Work with the Direct Services Director to develop programmatic budgets.
  • Assist Grants Manager in grant writing process including proposals and reporting.
  • Provide programmatic leadership for programs and activities in accordance with the strategic plan.
  • Assess annual and quarterly performance metrics to ensure alignment with the strategic plan and adherence with program budget.
  • Collect, analyze, and interpret program data to track outcomes, measure impact, and inform continuous program improvement and implementation strategies.
  • Contribute to the organizational health and strategic direction of THP as a member of the Leadership Team.
  • Participate in weekly Leadership Team meetings.
  • Report regularly to the Director of Direct Services and collaborate to ensure program fidelity, effectiveness, and sustainability.
  • Collaborate with other managers across The Health Partnership’s programs.
  • Engage in cross-organization efforts, connecting project work to the broader Partnership.
  • Other duties as assigned.
  • Hire, coach, and develop staff so they deliver on their key accountabilities, with high levels of employee engagement.
  • Work with HR/Operations Director to oversee employee recruitment, onboarding, and retention processes.
  • Supervise a core group of team members, monitor staff schedules, provide weekly individual and team check-ins.
  • Promote organizational culture that encourages top performance and high morale guided by THP’s core values of Trust, Collaboration and Innovation.
  • Provide ongoing training opportunities for staff and support them in their professional growth.
  • Ensure team members know and understand team objectives and expectations and work. together to achieve them through strong written and verbal communication skills.
  • Increase THP’s positive impact on our communities by enhancing collaborative partnerships across programs, the care coordination team, and with community partners.
  • Form trusting, collaborative relationships with community partners.
  • Develop a strong referral network for care coordination clients.
  • Identify new opportunities to partner with local and regional organizations and coalitions
  • Collaborate with partner organizations to maintain inter-organization communication and cooperation.
  • Attend multi-disciplinary meetings and community meetings.
  • Participate in community events as needed.
