Requirements

  • High School Diploma or GED Equivalent required
  • 2 to 5 years’ experience in population health and underserved populations.
  • Electronic Medical Record competency and proficiency.
  • Effective interpersonal skills and an understanding of the interdependent roles of various allied health professions.
  • Commitment to social justice and health equity.

Nice To Haves

  • Associate degree or higher in a relevant subject area such as public health, nursing, health administration, social work, business administration preferred; equivalent combination of education and relevant work experience may be considered.
  • Bilingual fluency in Spanish strongly preferred.

Responsibilities

  • Oversees the development and daily operations of care coordination services.
  • Leads care management initiatives focused on improving patient outcomes, advancing population health, and reducing healthcare costs through data analytics, policy development, quality improvement, and cross-department collaboration.
  • Supports high-risk patients by ensuring coordinated, comprehensive care across healthcare settings and promoting best practices to improve continuity of care and close healthcare gaps.
