Lead day-to-day customer service and care coordination for skilled nursing facilities. Coach and support a team of Care Coordinators, helping them grow and succeed. Serve as the primary point of contact for facility partners, handling questions and escalations. Build strong relationships and identify opportunities to expand services. Collaborate with nursing staff, healthcare professionals, and internal teams to support patient care. Review medical records and care plans to guide coordination and decision-making. Oversee communication channels to ensure timely follow-up and resolution. Maintain accurate documentation using EMR systems and digital tools. Educate facility partners on services, programs, and resources. Ensure compliance with healthcare regulations and company policies. Stay current on tools and programs, sharing knowledge with your team and partners.
