About The Position

The Care Coordination Program Coordinator will receive direction from the Program Director and will be responsible for training, providing direction to Care Coordinators, assisting the PD and ensuring the over-all quality/effectiveness of Care Coordination Wraparound services at all locations.

Ability to observe and accurately report the functioning of children and families.
Ability to handle stressful situations.
Must meet Care Coordinator training and certification requirements established by DHMH
Must complete trainings required by CSA and BHA, including CASII, ESCII, and CANS
Must complete Wraparound certification

Responsibilities

Supports Care Coordination Program Director
Maintain a caseload of no more than tec (10) Care Coordination clients.
Provides direction to Care Coordination employees at multiple locations.
Provides case management services in accordance to the Service Provision Level I or Level II.
Ensures case management services are coordinated and does not duplicate activities provided as part of institutional services and discharge planning activities.
Train new Care Coordinators at multiple locations.
Travel between locations as needed.
Process initial authorizations as needed.
Participate in agency events and assist with community outreach as needed.
Assist in program quality improvement.
Assist in preparing weekly unit sheets.
Maintain program compliance with network and regulatory standards.
Completes reports as needed.
Support preparation and maintenance for Care Coordination CARF Accreditation and facilitate the coordination of inter-agency program service delivery.
Ensure the delivery of quality and appropriate services to clients through supervision, adherence to COMAR regulations, and all policies and procedures, but not limited to, team meetings and case reviews.
