Pathways to Success is a highly structured program implemented by HFS for individuals under the age of 21 that are Medicaid eligible and meet specific criteria. This program offers intensive case management and full wraparound services to eligible clients and their families. The Care Coordinators link families to traditional outpatient services as well as Pathways-specific services. The Coordination and Support: High Fidelity Wraparound (CCSW) role is primarily responsible for facilitating the care coordination process for children with mental health diagnoses and their families. This involves leading Child and Family Team Meetings, coordinating with professionals, and assisting the child in achieving their goals. CCSW services are provided to children in Tier 1, with Care Coordinators managing an average caseload of 10 families, never exceeding 12. The CCSW supports families in developing a positive future outlook and utilizing a strength-based empowerment model to enhance the child's functioning at home, school, and in the community.
