About The Position

At NorthBay Health, the Cardiovascular Program Coordinator is an RN who provides leadership across the continuum to promote excellence in cardiovascular patient care. This person has responsibility and accountability for operational design, program implementation, policy development, and population management of cardiovascular patients (Acute Coronary Syndrome (ACS), heart failure, and other cardiac or vascular diagnoses). The Cardiovascular Program Coordinator’s role is to promote the provision of effective, high quality patient care to cardiovascular patients across the continuum with a collegial and interdisciplinary focus. The following responsibilities are included: coordination of fiscal and other resources, compliance with regulatory and professional standards, and fostering of interdisciplinary, collaborative relationships within the cardiology department and between other hospital departments. He/she will seek and provide educational programs for staff, physicians and community pertinent to the cardiovascular patient population served. Other responsibilities include coordination with the appropriate EMS and county agencies related to best practice responses and scene transports for patients experiencing cardiac emergencies, and coordination of quality and performance improvement initiatives. The Cardiovascular Program Coordinator works collaboratively with the ED Chest Pain Center Medical Director, Medical Director-Cardiology, Medical Director-Interventional Lab, the Sr. Director – Critical Care & Cardiopulmonary Services, the clinical, and operational team to ensure that all patient care activities related to patients with cardiac-related conditions are standardized and based on current best practices. The Cardiovascular Program Coordinator ensures continuous departmental and organizational compliance with accreditation standards as established by the ACC and other applicable accrediting and regulatory bodies. The Cardiovascular Program Coordinator is responsible for data collection and analysis activities surrounding the cardiac and vascular programs, and other cardiovascular focused performance improvement activities.

Requirements

  • Graduate from an accredited school of registered nursing.
  • Bachelor’s Degree in Nursing or related health care field is required.
  • Master’s Degree in Nursing or related health care field is required or required to be in progress.
  • One of the degrees must be in nursing.
  • Current California RN Licensure.
  • Current BLS and ACLS certification required.
  • Minimum 3 years of experience involved in the direct care of the cardiac patient in either the ED, Cath Lab, Cardiology, CCU or Telemetry.
  • Excellent oral and written communication skills required.
  • Must possess high professional standards.
  • Must be well organized, be able to set priorities, work independently and is an effective team member.
  • Word Processing skills required.

Nice To Haves

  • Certification by the American Nurse Credentialing Center preferred.
  • Working knowledge of population management preferred.
  • Previous experience in an accredited chest pain center preferred.
  • Statistical/data management skills preferred.
