Supports the service line goal of completing the Child and Adolescent Needs and Strengths Assessment (CANS) on eligible youth to determine OhioRISE eligibility. Completes the telephone-based CANS assessments during clinic operating hours and may complete in person assessments if the need arises. Completes CANS assessments, and other appropriate mental health screenings via telephone as the primary mode of assessment or in person if appropriate. Completes required charting and clinical documentation in accordance with NCH and applicable program standards. Completes training and passes required test to become a registered CANS assessor. Works collaboratively with other members of the team and assists with a warm handoff to treatment team if needed. Escalates to the Behavioral Crisis Center if a crisis occurs during the assessment provides and supports, including completion of a transition plan. Maintains currency in required training and familiarity with the research and literature in areas of expertise. Refers patients to appropriate behavioral health programming as needed.
