About The Position

The Campus Home Care Nurse assists with the overall planning, coordination, provision, supervision, and evaluation of clinical services for Osborn Home Care within The Osborn’s campus. As a representative of Osborn Home Care and the organization, the nurse is expected to perform essential job functions professionally and with a positive attitude, and to support the organization’s mission, vision, resident Bill of Rights, policies, and procedures.

Requirements

• Active New York State RN license.
• CPR certification.
• Three to five years of home care experience.
• Ability to work with others.
• Computer literacy and knowledge of electronic medical records (EMR).

Nice To Haves

• Bachelor of Science in Nursing (BSN).

Responsibilities

• Assist with coordinating health-related services for clients and families.
• Perform initial assessments of newly referred clients to assess care needs.
• Obtain appropriate and timely initial and updated physician orders.
• Develop and implement Plans of Care and treatment plans in conjunction with physician orders.
• Provide ongoing teaching to clients and families as needed to support health and wellness outcomes.
• Update medical records as appropriate.
• Supervise home health aides (HHAs).
