About The Position

Placement Partners provides person-centered case management services, as well as community-based vocational and residential services to adults with developmental disabilities, TBI, and others requiring assistance to work and live independently. As a Case Manager, you’ll spend most of your days working independently from home, coordinating services, completing documentation, communicating with clients, families, providers, and county teams, and monitoring client support plans. Your day will involve organizing your caseload, problem-solving client needs, responding to emails and calls, completing assessments and reports, and ensuring services are meeting each client’s needs and regulatory requirements. You’ll also complete client visits and team meetings in person, while managing your caseload and ensuring services are properly coordinated and documented.

Requirements

  • Bachelor’s degree, with emphasis in Social Services, psychology, human services, or other fields related to the education or treatment of people with developmental disabilities or related conditions.
  • Up-to-date Auto Insurance
  • Maintain an acceptable motor vehicle report

Nice To Haves

  • Previous 1-year work experience providing case management services to persons with developmental disabilities is preferred, but not required
  • License in Social Work preferred, but not required

Responsibilities

  • Coordinate services and supports for a caseload of approximately 40–50 clients
  • Develop and monitor person-centered support plans, goals, and outcomes
  • Connect clients and families with appropriate services, providers, and community resources
  • Coordinate with clients, families, providers, counties, and other support team members
  • Advocate for clients and assist with resolving service-related needs
  • Complete assessments, case notes, reports, and required documentation
  • Monitor service utilization and ensure services remain appropriate and compliant
  • Respond to client and team communications and address emerging needs
  • Maintain accurate records and complete quality assurance requirements
  • Follow Minnesota DHS regulations and company policies and procedures
  • Manage your schedule and caseload independently while meeting productivity and documentation expectations
