Placement Partners provides person-centered case management services, as well as community-based vocational and residential services to adults with developmental disabilities, TBI, and others requiring assistance to work and live independently. As a Case Manager, you’ll spend most of your days working independently from home, coordinating services, completing documentation, communicating with clients, families, providers, and county teams, and monitoring client support plans. Your day will involve organizing your caseload, problem-solving client needs, responding to emails and calls, completing assessments and reports, and ensuring services are meeting each client’s needs and regulatory requirements. You’ll also complete client visits and team meetings in person, while managing your caseload and ensuring services are properly coordinated and documented.
