Expand lines of service as defined by management by developing and retaining referrals from physicians and other key non-physician referral sources. This role involves collaborating closely with hospital discharge planners, social workers, case managers, physicians, management, and education staff to generate referral sources. The representative will develop strong relationships with key referral sources, identify opportunities for partnering and growth, and manage leads to ensure an efficient admissions process. The goal is to increase the number of referral sources within a facility, educate patients, families, and the community on programs and services, and coordinate the completion of the admission process. This role requires frequent travel within the community to schedule meetings and develop contacts, document referral sources in a contact management system, and develop events and educational forums. Maintaining existing referral relationships and utilizing analytic tools like Salesforce for planning and evaluation are crucial. The representative will also act as a liaison in professional forums, communicate the facility's mission, and create outreach initiatives through various channels to ensure regular communication with referral sources.
