About The Position
The Bloodless Medicine Care Coordinator coordinates a high-quality and consistent standard of interaction with patients and care team members within Jefferson Health/Abington. The liaison will serve as an advocate, educator, and facilitator for patients requiring blood management. Collaborates with care team to facilitate the needs of the patient. Serves as liaison and resource to patients, providers, families, and patient care colleagues.
Requirements
- Associate’s Degree
- 2 years experience in blood management
Responsibilities
- Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
- Conducts daily patient rounds and chart review, generates bloodless program inpatient report daily, assists with data collection, compiles monthly activity, referral and statistical reports.
- Serves as liaison and resource to patients, providers, families, and colleagues regarding blood management.
- Assists patients in completion of advanced directives related to alternatives to blood utilization. Meets with patients to discuss blood management options.
- Assists with hospital-hospital transfers, conducts follow up calls on discharged patients, provides after hours on-call and call back coverage to offer 24/7 coverage
