Provides case management services to eligible consumers as defined by DPW Title 55 Code Chapter 5240. The Blended Case Manager provides case management services to adolescents, children, and adults who suffer from a serious mental illness or emotional disturbance. An assigned blended case manager works with the client and families to access, link, coordinate and monitor needed services as well as to provide support and advocacy to assist clients in maintaining in their home or community. BCMs will be available to meet with consumers during normal business hours, as well as in the evenings and on weekends when scheduled, to meet the needs of the individual or when an individual is experiencing a crisis event. An on-call BCM will also be available via the on-call hotline 24 hours a day, 7 days a week. Mileage reimbursement is available at the state set rate! The starting rate is $21.54/hour but may start higher depending on experience and education. The On-Call hourly rate is $21.54/hr. and we pay a stipend of $250.00 per week when covering on call for one full week.
