The Bilingual Perinatal Navigator supports pregnant individuals and families through pregnancy, postpartum recovery, and the early stages of their child’s life. The role helps patients access care, education, and community support, reduces barriers to care, and encourages sustainable health behavior change through health education, goal setting, motivational interviewing, and other evidence-based techniques. The navigator works with an integrated interdisciplinary clinic and care team, supports Legacy’s Walk-In Pregnancy Testing services and Peripartum Navigation program, and helps patients connect with Eligibility services, coordinate New OB, postpartum, and newborn appointments, and access resources for healthy families. The position includes point-of-care testing, health screenings and risk assessments, patient outreach, appointment coordination, culturally sensitive education, patient retention efforts, community resource referrals, and documentation in Legacy EMRS. It also involves collaboration with internal and external partners, participation in meetings and training, and meeting assigned performance objectives, including applicable grant or contract deliverables. Funding may be partially or fully supported by grants.
