About The Position

Marion County Health & Human Services is seeking an organized, motivated, flexible, and person-centered Bilingual Services Coordinator to join our Intellectual and Developmental Disabilities (I/DD) team. In this role, you will provide case management services to individuals experiencing intellectual and developmental disabilities, working collaboratively with individuals, families, service providers, and community partners to coordinate disability-related services across a variety of home and community-based settings. GENERAL STATEMENT OF DUTIES Facilitates intake and eligibility determination for applying individuals; conducts independent functional needs assessments; and, plans and carries out case management functions and other related work as required. Primary responsibility of this class is case management, facilitating intake and eligibility determination and administration of functional needs assessments.

Requirements

  • Five years of education in a related field and/or paraprofessional experience/training consisting of serving individuals with a primary diagnosis of developmental disabilities within a residential, employment or case management service at a management or case management level.
  • Knowledge of the public service system for developmental disability services in Oregon.
  • Any satisfactory combination of work, education, training, or experience relevant to the position, as determined by Marion County.
  • Fluent bilingual Spanish/English skills are required.
  • Must possess a current driver's license in the applicant's state of residence and an acceptable driving history.
  • Must not be excluded from participation in the federal health care programs (Medicaid, Medicare and other federally funded programs that provide health benefits).
  • Must not be excluded from participating in federal procurement (Federal Acquisition Regulation) and non-procurement activities (Executive Order No. 12549).
  • Knowledge of and ability to act professionally in regard to a code of ethics and confidentiality.
  • Working knowledge of developmental disability concepts, public service system for intellectual and developmental disability services in Oregon, including the Oregon Office of Developmental Disability Services structure, and related agency functioning; other social service agencies in the community.
  • Advanced skill in coordinating and/or providing services to individuals with intellectual or developmental disabilities or another similar population.
  • Ability to establish good working relationships with other employees, other agencies, and the public.
  • Ability to prepare concise reports and document individual services in a timely and professional manner.
  • Drives a motor vehicle to home visits and maintains an acceptable driving history.

  • Must complete the US Department of Homeland Security’s I-9 form confirming authorization to work in the United States within three days of hire.

Responsibilities

  • Facilitates intake and eligibility determination for applying individuals.
  • Conducts independent functional needs assessments.
  • Plans and carries out case management functions.
  • Provides services to monolingual Spanish-speaking individuals.
  • Drives vehicle to and from community meetings and home visits.
  • Uses county issued communication tools including smart phones and computers during work time.
  • Offers choice, annually and as needed, of all available service, provider and setting options.
  • Facilitates development of Individual Support Plan for all individuals receiving K plan funded services in their own home, family home, or in a licensed or certified residential setting.
  • Authorizes services and supports as described in Oregon Administrative Rule 411-415-0070.
  • Completes enrollment, termination, and transfer on required forms.
  • Facilitates or participates in an annual functional needs assessment process per current ODDS procedure.
  • Completes annual summary for all individuals not enrolled in any K plan funded service (case management only).
  • Participates in service planning by attending Individual Educational Plan meetings for students.
  • Mandatory to attend IEP meetings for individuals aged 15 and older to complete transition planning.
  • Maintains an accurate client record including Marion County generated documents and documents received from other individuals/agencies.
  • Documents all targeted case management activities in a progress note per Marion County policy.
  • Monitors services and supports for individuals enrolled in case management as described in Oregon Administrative Rule 411-415-0090 and documents per ODDS guidelines.
  • Reports Serious Events per published guidelines.
  • Provides information, timely referral, and assistance to individuals regarding available services to meet assessed needs.
  • Coordinates with community partners regarding provision of required supports from Community Developmental Disabilities Program to ensure safety and support needs are adequately met.
  • Attends staff meeting, vacancy meeting, and team meetings.
  • Performs other duties as assigned by your supervisor.
  • Makes a reasonable effort to report to work in the event of an emergency.
  • May be required to perform duties outside of their regularly assigned duties.
  • May be requested to work an extended or flexible schedule.
  • Completes Incident Command System 100 and 700 on-line or in-person training.

  • An additional 5% will be added to the hourly wage as compensation for bilingual Spanish/English skills.
  • Full-time position, which is eligible for overtime.
  • Marion County is a qualifying public employer for the Public Service Loan Forgiveness Program.
  • Full-time employees working at the County may qualify for forgiveness of the remaining balance on Direct Student Loans after 120 qualifying monthly payments under a qualifying repayment plan.
