Felton Institute is seeking a Bilingual Case Manager for its Justice Services Division, specifically for the Young Adult Court (YAC) program in San Francisco. YAC serves eligible young adults aged 18-24, operating within the larger Collaborative Court system. This program involves collaboration with various agencies including the Superior Court, Office of the District Attorney, Office of the Public Defender, Department of Public Health, Adult Probation Department, Department of Children, Youth and their Families, and Felton Institute. The Bilingual Case Manager will work with a clinical team to provide comprehensive, strengths-based, and client-centered support to justice-involved transition-age youth (TAY). The role requires adaptability in a fast-paced environment and a commitment to harm reduction principles. The Case Manager will develop individualized Wellness Care Plans with clients, track progress, and facilitate access to various services such as physical health care, benefits acquisition, housing, employment, and social services. They will also assist with family needs, parenting support, crisis intervention, safety planning, stress management, and developing healthy coping strategies. The position fosters a welcoming, safe, supportive, and culturally affirming environment. Key duties include maintaining accurate documentation, managing data entry, generating reports, and ensuring continuity of care.
