Functions as a liaison and advocate for patients between their care team, specialty care, and/or other relevant staff and assists patients in navigating referrals to community resources and specialty care. Do you have your Bachelor in Psych, Social Work or Sociology and looking to get experience in mental health? Are you passionate about helping people continue to engage with services dealing with trauma, anxiety and depression? Do you have experience with the criminal justice system? Do you want to develop some case management skills? Are you a person that does not miss details? Is building relationships with clients easy for you? You will report to the Program Supervisor.
