About The Position

This Case Management Analyst position is with Aetna’s Long-Term Services and Supports (LTSS) team and is a field-based position. Candidates must be located in Miami-Dade County, specifically in zip codes 33002, 33010, 33011, 33012, 33013, 33015, 33016, 33018, 33142, or 33147. This role will require 50-75% travel for face to face visits with members in Miami-Dade County. Schedule is Monday through Friday, 8:00am - 5:00pm, Standard Business Hours. We are seeking self-motivated, energetic, detail oriented, highly organized, tech-savvy Case Management Analyst to join our Case Management team. Requirements

  • Bilingual: (English & Spanish).
  • Must reside in zip codes 33002, 33010, 33011, 33012, 33013, 33015, 33016, 33018, 33142, or 33147.
  • Willing and able to travel 50-75% of their time to meet members face to face in Miami-Dade County and surrounding areas in Florida.
  • 1+ year of experience in behavioral health or a long-term care setting.
  • Computer proficiency in Microsoft Word, Excel, and Outlook required.

Responsibilities

  • Case Management Analyst is responsible for telephonically and/or face to face assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member's overall wellness.
  • Case Management Analyst will effectively manage a caseload that includes supportive and medically complex members.
  • Case Management Analyst will develop a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member's overall wellness through integration.
  • Case Management Analyst will determine appropriate services and supports due to member's health needs; including but not limited to prior authorizations, coordination with PCP and skilled providers, condition management information, medication review, community resources and supports.
