About The Position

This position, a Case Management Coordinator, is part of the Case Management team and offers a $5,000 New Hire Bonus. Candidates must reside in Miami Dade county, specifically in the Hialeah and Miami Lakes area, and the role requires 50-75% travel for face-to-face visits with members in Miami Dade. The schedule is Monday-Friday, 8:00am-5:00pm, offering autonomy, paid holidays, and flexibility in coordinating member care. The role provides a competitive salary and full benefits.

Requirements

  • Bilingual, English, and Spanish.
  • Willing and able to travel 50-75% of their time to meet members face to face in Miami Dade county and surrounding areas in FL.
  • Must reside in Hialeah and Miami Lakes Florida or Surrounding Areas.
  • 1+ year of experience in behavioral health or a long-term care setting.
  • Computer Proficiency in Microsoft Word, Excel, Outlook required.
  • Bachelor's degree required, preferably in social work or related field.

  • Effective communication skills, both verbal and written.
  • Managed Care experience.
  • Case management and discharge planning experience.

Responsibilities

  • Telephonically and/or face to face assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member's overall wellness.
  • Effectively manage a caseload that includes supportive and medically complex members.
  • Develop a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member's overall wellness through integration.
  • Determine appropriate services and supports due to member's health needs; including but not limited to: Prior Authorizations, Coordination with PCP and skilled providers, Condition management information, Medication review, Community resources and supports.
