The role involves 50-75% travel for face-to-face visits with members in Miami-Dade County. The schedule is Monday through Friday, 8:00am - 5:00pm, standard business hours. The ideal candidate is a self-motivated, energetic, detail-oriented, highly organized, tech-savvy Case Management Analyst to join the Case Management team. The opportunity includes a competitive salary and full benefits, promoting autonomy through a Monday-Friday schedule, paid holidays, and flexibility in coordinating member care.

Requirements

  • Bilingual: (English & Spanish)
  • 1+ years of experience in Behavioral Health or a Long-Term Care setting
  • Computer proficiency in Microsoft Word, Excel, Outlook required
  • Must reside in Miami-Dade County, FL (Hialeah, zip codes 33010, 33012, 33013, 33014, 33015, 33016, 33018)
  • Willing and able to travel 50-75% of their time to meet members face to face in Miami-Dade County and surrounding areas in Florida

Responsibilities

  • Telephonically and/or face to face assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member's overall wellness
  • Effectively manage a caseload that includes supportive and medically complex members
  • Develop a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member's overall wellness through integration
  • Determine appropriate services and supports due to member's health needs; including but not limited to prior authorizations, coordination with PCP and skilled providers, condition management information, medication review, community resources and supports
