The role involves 50-75% travel for face-to-face visits with members in Miami-Dade County. The schedule is Monday through Friday, 8:00 AM - 5:00 PM, standard business hours. The company seeks self-motivated, energetic, detail-oriented, highly organized, tech-savvy Case Management Analysts to join their team. Case Management Analysts are responsible for telephonically and/or face-to-face assessing, planning, implementing, and coordinating all case management activities to evaluate members' medical needs and facilitate overall wellness. They will manage a caseload of supportive and medically complex members, developing proactive courses of action to enhance short and long-term outcomes and overall wellness through integration. Analysts will determine appropriate services and supports based on member health needs, including prior authorizations, coordination with PCPs and skilled providers, condition management information, medication review, and community resources.

Requirements

  • Bilingual: (English & Spanish)
  • 1+ years of experience in Behavioral Health or a Long-Term Care setting
  • Computer proficiency in Microsoft Word, Excel, Outlook required
  • Must reside in Miami-Dade County, FL (Hialeah, zip codes 33010, 33012, 33013, 33014, 33015, 33016, 33018)
  • Willing and able to travel 50-75% of their time to meet members face to face in Miami-Dade County and surrounding areas in Florida
  • Bachelor's degree required, preferably in Social Work or related field

Responsibilities

  • Telephonically and/or face to face assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member's overall wellness
