About The Position

This role is for a Behavioral Health Social Worker in the Eating Disorder Unit at Ascension Saint Joseph in Chicago, IL. The position is PRN, with day shifts from 8:30 am to 4:00 pm. The salary range is $40.00 - $42.00 per hour. The role involves creating care plans, monitoring patient care, advocating for resources, and maintaining communication with supervisors and other health providers.

Requirements

Clinical Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
Social Worker specializing in Clinical Practice credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
Social Worker credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
MSW, LSW required

Nice To Haves

Weekday and weekend coverage
Float to all 3 units, but primarily for the EDU to help with PTO and coverage needs

Responsibilities

Create plan for care across the continuum, integrating patient/family preferences and values.
Monitor patient care through assessments, evaluations and/or patient records.
Advocate for resources and removal of barriers.
Maintain ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.
