About The Position

Brevard Health Alliance, Brevard County’s only Federally Qualified Health Center, is currently recruiting for a Behavioral Health RN to join us in the heart of Brevard County’s Space Coast. Since 2005 our focus has been on putting the “community” in Community Health while delivering healthcare to more than 60,000 unique patients annually. The Behavioral Health RN provides high-quality, patient-centered care within a collaborative, team-based model at our FQHC. This role supports the Behavioral Health Clinical Team by coordinating care, performing clinical assessments, administering medications, and ensuring continuity across services. The RN works closely with patients and care teams to reduce barriers, improve outcomes, and promote wellness within underserved communities. This position has a shared reporting structure to the Director of Behavioral Health and Director of Clinical Operations. The Registered Nurse (RN) provides clinical oversight within their specialty and manages a panel of patients to support positive health outcomes through in-person and telephonic case management. The RN collaborates with providers to perform or assist with patient assessments, MAT in office inductions, injection clinic, care management, and contributes to the development and evaluation of comprehensive care plans. This role ensures coordinated, efficient care by addressing physical, psychosocial, and environmental needs through an interdisciplinary approach. The RN fosters strong team relationships, advocates for patients and colleagues, and serves as a clinical leader and role model. They are accountable for maintaining clinical competence and adhering to established standards of care and professional practice.

Requirements

  • Graduate from a program accredited by the National League for Nursing Accrediting Commission (NLNAC), or the Commission on Collegiate Nursing Education (CCNE) and has met licensure requirements for practice.
  • Associate Degree in Nursing, Bachelor of Science in Nursing, preferably from an accredited university.
  • Clear and active Florida/compact Nursing License
  • Minimum of four (4) years healthcare experience with two (2) years recent experience in an outpatient/ambulatory/behavioral health care setting preferred
  • Current BLS/CPR certification from the American Heart Association or the American Red Cross
  • Proficient in Electronic Medical Records (EMR)
  • Demonstrates strong oral and written communication skills

  • If a current clinical BHA employee earns a registered nurse license during employment, the experience qualification requirement may be waived based upon leadership discretion.

Responsibilities

  • Uses the nursing process to formulate safe and effective care based on each individual’s unique needs (Including psychosocial, spiritual, cultural, biophysical, educational, and age specific needs)
  • Deliver nursing care in alignment with patient-centered medical home (PCMH) principles, emphasizing respect, communication, and cultural sensitivity.
  • Support specialty providers through patient intake, assessments, procedures, and follow-up coordination.
  • Leads MAT in-office inductions including assessing (COWS), patient monitoring, and collaborating with the physician. Provides thorough follow up and care management post-induction.
  • Triage patients by phone and in-person, ensuring timely response to clinical needs.
  • Coordinate specialty referrals and diagnostic testing; follow up on results and care plans.
  • Provide patient education on disease processes, medications, and care plans, tailored to individual needs and literacy levels.
  • Administer medications and treatments according to standing orders and provider instructions.
  • Document all encounters and interventions accurately and in real-time within the EHR.
  • Collaborate with care team members including primary care, behavioral health, specialty services, and case management to ensure whole-person care.
  • Participate in quality improvement initiatives and care team huddles to enhance patient experience and outcomes.
  • Uphold FQHC mission by advocating for vulnerable populations and addressing social determinants of health.
  • Able to perform and carry out projects as delegated by the Site Manager or Director of Clinical Operations or designees
  • Takes ownership and represents Brevard Health Alliance’s mission and organizational goals established by the executive committee. These include, but are not limited to: Productivity, Quality, Patient Satisfaction, and Employee Retention. It is the duty of each BHA employee to incorporate these goals into all daily work routines and strive to achieve and surpass all goals by working as a team. The mission and goals of BHA will be incorporated into each associate’s performance plan to be evaluated annually.
  • Travel within different BHA clinics and will reimburse your local travel, per BHA policy and the current IRS reimbursement rate.
