Behavioral Health Provider (LCSW, LMFT, LPC)

InnovAge•Orlando, FL

About The Position

InnovAge PACE provides customized healthcare, social engagement, and exercise to older adults to help them remain independent and live in their homes. Participants receive most of their healthcare and social support at a local InnovAge center, with a team of healthcare experts and a personalized care plan. The Behavioral Health Provider (BHP) helps develop and deliver high-quality, clinically appropriate, team-based behavioral healthcare. The role provides initial assessments, clinical interventions, referrals for participants and families, and treatment recommendations. Physical attendance at an InnovAge center is essential, with travel to meet participants outside the center as needed. This is a Monday-through-Friday position with no on-call or weekend hours required. The position involves clinical care (70%) and administration (30%). The BHP works with participants, caregivers, the Interdisciplinary Team (IDT), and external providers to assess behavioral health needs, provide interventions, coordinate care, and support continuity of treatment. The role calls for flexibility in ambiguous and changing situations, constructive use of feedback, and the ability to adapt interpersonally to participant and IDT needs.
