About The Position

Oak Street Health’s Behavioral Health Specialist provides care management and evidence-based short-term psychotherapy to patients with behavioral health needs. The role supports behavioral healthcare across a region of Oak Street Health primary care clinics in collaboration with Care Teams and a psychiatric consultant. The specialist assesses patient needs, makes referrals to appropriate behavioral health resources and specialists, and provides short-term counseling and evidence-based treatment when indicated. Remote work days may be available based on achievement and maintenance of program metrics.

Requirements

Master’s degree in mental health counseling or the healing arts from an accredited school, such as Social Work, Counseling, or Marriage and Family Therapy.
Highest clinical level of license, or local equivalent.
Experience screening for common mental health and/or substance use disorders.
Experience assessing and developing treatment plans for common mental health and/or substance use disorders.
Familiarity with brief, structured intervention techniques, such as Motivational Interviewing, Behavioral Activation, Problem Solving, and Cognitive Behavioral approaches.
US work authorization.

Responsibilities

Collaborate with primary care teams to identify appropriate patients for referral to the Oak Street Behavioral Health Program.
Screen and assess patients for common mental health and substance use disorders.
Develop care plans and provide or facilitate referrals to Oak Street’s psychiatric consultant and external resources.
Provide short-term counseling and evidence-based treatments.
Coordinate care with Oak Street’s psychiatric consultant, including warm handoffs for telehealth visits and collaboration on care plans.
Build relationships with primary care teams and communicate changes in behavioral health care plans.
Document patient encounters and counseling sessions accurately and promptly in Oak Street’s electronic medical record.
Participate in regional and organizational efforts to advance the Behavioral Health Program through workshops, feedback sessions, and surveys.
Possibly cover multiple clinics, depending on clinic maturity and panel sizes.
Perform other duties as assigned.
